gstack QA: Test and Fix
Tests a site, API or CLI, then fixes the bugs it finds in severity order, each with a regression test and its own commit.
Runs git commit for each verified fix, and asks you to commit or stash existing changes if the tree is dirty.

Core rule
Reproduce each defect in a regression test before repairing it, and commit only verified fixes.
Execution Phases
Phase 01Setup
Parse target, tier and mode; if the tree is dirty, let the user choose commit, stash or abort; create the report directory.
Phase 02Baseline QA
Run exploratory probes on the target, keep browser and functional results separate, and report baseline findings before fixing.
Phase 03Triage
Sort issues by severity, apply the selected tier, and mark lower-tier or unfixable issues as deferred.
Phase 04Fix loop
Reproduce, write the regression test, make a minimal fix, re-test and commit each one; stop after two reverts, with a hard cap of 50 fixes.
Phase 05Final QA and report
Re-run affected checks, compute the health score, write the QA report, and update TODOS.md.
